Accident Waiver and Release of Liability Form

Family Escape Rooms LLC release and waiver of liability, assumption of risk, indemnity agreement with parental consent. By signing this waiver:

I understand that Family Escape Rooms LLC, and its staff are not responsible for any personal belongings.
I certify that I am qualified, in good health, and in proper physical and mental condition to participate in activities that may feature UV lighting, or dimly lit areas.
I certify that Family Escape Rooms LLC is not liable for any incidental, consequential, or punitive damages of any kind relating to any damage to property or injury to myself.
I understand that if I selected a Virtual Reality (VR) experience, that the equipment has a manufacturer recommended age of 13 and older.
I understand and agree that during my experience at Family Escape Rooms LLC, I am under surveillance. Damage to props and property will be charged to the credit card on file.
If applicable, as supervising adult I understand that it is my responsibility to communicate the details to, and sign this waiver on behalf of, any child(ren) within my party.
